摘要:
目的 探讨应用Amplatzer 蘑菇伞器治疗小儿动脉导管未闭(PDA) 的临床疗效及价值。方法 本组共68 例,年龄8 个月～ 12 岁(平均6. 4 岁) , 体重6. 5～ 30 (平均18. 2) kg, PDA 直径为3～ 8 mm , 术中行右心导管检查取血, 测压, 术后5～ 15 min 行降主动脉造影, 测肺动脉压, 主动脉压。术后1 月, 3 月, 6 月分别行X 线及超声心动图检查, 了解有无残余分流及封堵器位置。结果 本组68 例均应用Amplatzer PDA 磨菇伞器治疗, 成功率100% , 疗效满意, 无并发症, 术后3 d 行超声心动图检查显示无1 例分流。结论 应用Amplatzer 蘑菇伞器治疗PDA 创伤 小, 操作简单, 使用广泛, 成功率高, 临床疗效满意。
Abstract:
AIM To assess the safety and efficacy of the Amplatzer ductaloccluder in the treatment of patent ductus arteriosus (PDA) in children. METHODS A total of 68 patients underwent transcatheter occlusion of PDA at a medianage of 6. 4 years ( range 8 month～ 12 years) and at a median weight 18. 2 kg ( range 6. 5～ 30 kg). The diameters of ductus arteriosus were 3～ 8 mm.Aorta and pulmonary pressures were measured during and post implantation, and descending aorta imaging was performed 5～ 15 m in post implantation. X-ray and 2D-echocardiogram examinations were performed 1, 3 and 6 months after implantations to reveal the shunt and devices position. RESULTS Complete closure was achieved in all patients (100% closure rate) and no complications were observed. No residual shunt was found 1 month after implantation by 2D-echocardiogram.CONCLUSION Transcatheter occlusion of PDA using Amplatzer ductal occluder is safe, effective and easy to perform. It is a satisfactory alternative method in the treatment of PDA in children.
