摘要:
目的 应用Amplatzer 封堵器在高原地区治疗动脉导管未闭(PDA ) 并评价其疗效。方法 本组20 例, 年龄1.5～44 (22±14) 岁, PDA 最窄处内径5～14 (9±3)mm , 均用Amplatzer 封堵器介入治疗。结果 19 例成功, 1例严重阻力性肺动脉高压不适宜封堵。术后即刻降主动脉造影显示15例封堵完全无残余分流, 4例少量残余分流。1例术后1h 封堵器脱落入右肺动脉, 第三小时急诊外科手术关闭PDA 并取出封堵器。24h 彩色多普勒检查显示封堵18例患者均无分流, 随访2月～ 1.5 年, 无1例分流。结论 用Amplatzer 封堵器治疗PDA 是高原地区1种适应性强,安全有效的介入治疗方法。
Abstract:
AIM To evaluate the efficacy of Amplatzer occluder device in the management of patent ductus arteriosus (PDA) in plateau regions.METHODS Twenty aged from 1.5 to 44.0years with a mean of 22±14 years patients PDA, underwent attempted transcatheter closure of a PDA using the Amplatzer occluder device. The PDA diameter at its narrowest segment was (9±3) (rang 8 to 14)mm.RESUTS 19 of the 20 cases with PDA underwent successful attempted transcatheter closure with the Amplatzer occluder. One was aborted because of the resistant pulmonary hypertension. Angiography showed that 15 patients had complete immediate closure and 4 had small shunt 10 min after the procedures. The occluder dropped into the right pulmonary artery in one case, which was taken out through the emergency surgical operation and PDA was closed . The color Doppler within 24 h revealed complete closure in 18 patients. No residual shunt and PDA regurgitation after the complete closure were found in 2 to 18 months follow-up. CONCLUSION Anterograde transcatheter closure using the Amplatzer occluder device proved to be a safe and effective interventional therapy for PDA patients with a high success rate and immediate satisfactory results.
