摘要:
目的 研究在犬急性心肌梗死状态下,顺磁性造影剂Gd-DTPA首次通过心肌时纵向驰豫加权图像(T1WI)和横向驰豫加权图像(T2WI)对病变心肌(梗死心肌和/或缺血心肌)的显示特性及其与病理结果的关系。方法 选10只杂种犬,闭塞冠状动脉左旋支2.5~5.5 h,将Gd-DTPA(0.1 mmol·L-1·kg-1)注入左心房30~40 s后,离体心肌行T1WI和T2WI扫描,观察心肌的信号特征,并将T1WI,T2WI和绿化三苯四氮唑(TTC)显示的病变心肌范围进行相关性比较及t检验。结果 Gd-DTPA首次通过心肌时,T1WI病变心肌呈低信号,T2WI病变心肌呈高 信号,前者包括后者或TTC所示梗死心肌及缺血心肌,T1WI和T2WI间r=0.77,T1WI和TTC间r=0.85,T2WI与TTC间r=0.97,其中T2WI与TTC的x-±s间无显著差别(P>0.05)。结论 Gd-DTPA首次通过心肌时,T1WI所示病变心肌包括缺血心肌和梗死心肌,T2WI只显示梗死心肌,两者联合应用可客观评价心肌的血供和存活情况。
Abstract:
AIM To study the characteristics of the abnormal(infarct and/or ischemic) myocardium on FIRST-PASS Gd-DTPA enhanced T1WI and T2WI under the condition of the acute myocardial infarction,and their relationships with that of the physiologic result.METHODS:The LCx of 10 healthy mongrel dogs were occluded by 2.5~5.5 h. The hearts were then excised 30~40 s after the injection of Gd-DTPA(0.1 mmol·L-1·kg-1) into the left atrium.On T1WI and T2WI,the signals of the myocardiums werein vitroobserved,and furthermore,the areas of the abnormal myocardium were respectively compared among the results on T1WI,T2WI,and TTC. RESULTS:On the FIRST-PASS Gd-DTPA enhanced T1WI,the abnormal myocardium showed low signal,whereas the abnormal myocardium demonstrated high signal on T2WI. The extents of the abnormal myocardium on T1WI included ischemic myocardium except that on T2WI or TTC, the linear correlation coefficients among them were 0.77(T1WI and T2WI),0.85(T1WI and TTC),and 0.97 (T2WI and TTC) respectively,in which there is no difference between thex-±s of T2WI and TTC(P>0.05).CONCLUSION:On the FIRST-PASS Gd-DTPA enhanced MR imaging,the abnormal myocardium include ischemic and infarct areas on T1WI,and only infarct areas on T2WI correspondingly, accordingly,the blood supply and whether live or not of the myocardium can be well evaluated with the combined appliance of them.
