目的:了解磁共振成像(MRI)诊断心脏疾病的价值。方法:分析31例心脏疾病的MRI解剖结构和血流动力学改变,其中心包积液(PCE)11例,心包血肿(PCH)1例,主动脉瓣关闭不全(AI)12例,二尖瓣狭窄并发关闭不全(MS+MI)1例,肥厚型心肌病(HCM)3例,房间隔缺损(ASD)2例,ASD并发室间隔缺损(VSD)1例。结果:在SE序列,PCE在T1WI呈低信号(9例)和高信号(2例),T2WI呈高信号;PCH分别呈略高信号和略低信号;AI和MS+MI分别有左心室和左心室、左心房扩大;HCM 左心室心肌明显增厚,ASD缺损清楚,VSD显示不清。在cine-MRI上,AI和MS+MI分别在左心室和左心室、左心房内出现低信号区;HCM 的心肌收缩率下降,ASD和VSD分别在右侧房室出现低信号区。结论:MRI可以较好地显示心脏疾病的解剖异常和血流动力学状态的改变,多数疾病可明确诊断。
Abstract:
AIM: To find out the value of magnetic resonance imaging (MRI) in diagnosing the heart diseases. METHODS: the anatomic configurations and hemodynamics of 31 caseson MRI were analysed, including pericardial effusion (PCE,11cases), pericardial hematoma (PCH,1case), aortic valvular insufficiency (AI,12cases), mitral valvular stenosis and insufficiency (MS+MI,1 cases), hypertrophic cardiomyopathy (HCM,3cases), ASD (1case), and ASD+VSD (1case,).RESULTS: On SE sequence, PCE presented as low (9cases) or high (2cases) signal on T1WI, andhigh signal on T2WI; PCH presented as slightly high and low signal respectively; left ventricle (LV) and atrium (LA) were enlarged in AI and MS+MI; the markedly thickened myocardium of the LV were found in HCM; the defects were found in ASD but not in VSD. On cine-MRI, low signal were found in LV (AI) and LV+LA (MS+MI) respectively; the myocardial contractrate declined in HCM; low signal were found in right atrium (ASD) or ventricle (VSD) respectively. CONCLUSION: The anatomic configurations and hemodynamics of the heart can be clearly demonstrated on MRI, and most of the heart diseases can be correctly diagnosed.
