摘要:
目的 比较透析与非透析慢性肾功能衰竭患者并发急性心肌梗死的临床特征。方法 按是否接受维持性肾透析分组，比较其临床特点。结果 与非透析患者比较，透析患者入院诊断为急性心肌梗死的比例低，有胸痛患者比例少，肌钙蛋白阳性率高，心电图表现为ST段抬高者比例低，误诊率高；左室射血分数低，住院病死率高。结论 维持透析的慢性肾功能不全并发急性心肌梗死临床表现不典型，容易误诊，预后不良，病死率较高。
Abstract:
AIM To compare the clinical characteristics of dialysis and nondialysis patients with acute myocardial infarction. METHODS Retrospective analysis of the clinical characteristics of 185 chronic kideny disease patients with acute myocardial infarction was conducted. RESULTS Compared with the nondialysis patients, fewer dialysis patients were diagnosed as having myocardial infarction on admission and chest pain and ST elevation presented in fewer dialysis patients. Troponin was positive in more dialysis patients, LVEF was lower than nondialysis patients, and in-hospital death was much higher. CONCLUSION Dialysis patients hospitalized for AMI have poor outcomes and their mortality rate is higher.
