本文研究了由手术证实的356例选择性右室负荷过重型先心病儿童心电向量图。按病种共分9个类型。术前常规记录Frank导联心电向量图。测量和统计了F、LS及H面QRS环在各象限的面积比率,用本室相应年龄正常人心电向量图作对照组。进行了数理统计及诊断试验研究。结果表明:诊断先心病右室肥厚最有价值的指标走H2大于总面积15%、F2大于总面积30%及F1小于总面积50%。用QRs环面积法诊断右室肥厚的敏感性、特异性、准确性、阳性似然比及阳性诊断价值分别为60.0%~88.3%、92.3%~95.8%、90.9%~95.1%、8.7~20.5及89.7%~95.5%.试验证明,QRS环面积是诊断右室肥厚最简易而可靠的定量诊断方法。
Abstract:
In order to evaluate QRS loop areas in the diagnosis of right ventricular hypertrophy.We studied vectorcardiograms in 365 children with surgically confirmed congenital seleective right ventricular overload.The ratios of the QRS 1oop areas in various quadrants of F,LS and H planes were analysed and compared with those of normal children.The results showed that the valuable criteria for the diagnosis of right ventricular hypertrophy were:①the QRS loop area in the H2 (the right anterior quadrant of the horizontal plane)was greater than 15%of the total;②the QRS loop area in the F2(the right inferior quadrant of the frontal plane)was greater than 30%of the total;③the QRS loop area in the F1(the left inferior quadrant of the fronal plane)was less than 50% of the total.The sensitivity,specificity,accuracy,positive.1ikelihood ratio and positive diagnostic value in the 9 kinds of congenital heart diseases were 60.O%~88.3%,92.3%~95.8%,90.0%~95.1%,8.7%~20.5% and 89.7%~95.5%,respectively.It is concluded that the proposed vectorcardiographic criteria are simple reliable and quantitative for the diagnosis of right ventricular hypertrophy.
