摘要:
本文应用国产无创性心脏起搏监护仪,对42例缓慢型心律失常患者进行了胸壁皮肤电极心脏起搏,其中11例监测左、右心室压及主动脉和肺动脉压,23例抽取肺动脉血用Fick氏法测计心排量。各参数均与右室心内膜起搏对比。结果表明:无创性胸壁皮肤电极起搏确是一简便、决速、有效的临时起搏方法,可获得与心内膜起搏相似的血液动力学效果。
Abstract:
Forty-two cases of bradyarrhythmia underwent cardiac pacing with thoracic cutaneous elect trodes with the help of a domestic nontraumatic cardiac pacemaker monitor;the rate of Success was 100%,obtained with a R-wave inhibitted ventricular pacemaker and the current threshold of pacing was 15~72 mA(average value:32.14±17.79 mA).The hemodynamic changes of 11 cases were observed by left and right cardiac monitor catheter method,and the results showed that while the patients were undergoing cardiac pacing,the 1eft and fight ventricular systolic pressure,aortic systolic pressure and pulmonary arterial systolic pressure were higher by means of nontraumatic cardiac pacing with thoracic cutaneous eletrodes than those bV the method of endocardiac pacing.The cardiac output of 23 cases was tested by Fick method and the result indicated that the cardiac index has no statistic difference,as compared with the two above methods.Therefore,nontraumatic cardiac pacing with thoracic cutaneous electrodes is thought to be a simple,rapid and effective temporary cardiac pacing method.In addition,it has the same hemodynamic effect as that of endocardiac pacing.
