应用无创左右心功能同步检测仪测定原发性高血压患者234例的心功能参数与40例正常人比较。结果显示心肌缺血阈值下降; 左室射血分数、左室舒张末期压力、肺毛细血管平均嵌楔压、心肌耗氧量、左室舒张末期血量、左室射血阻抗和动脉弹性模量等较对照组升高(P<0.01)。原发性高血压的心脏早期病变主要表现为左室舒张功能障碍(35% ) , 其受损程度与年龄、病程无相关性。年龄60岁左右的原发性高血压 患者心率维持在55～ 69 次/min 范围内, 似有利于心肌供血。心功能检测对无症状心功能不全的早期诊断与防治具重要意义。
Abstract:
Investigation of cardiac functional capacity in 234 cases of essential hypertension (EH) campared with 40 normal subjects as controls was performed with a non-invasive synchronous ventricular function detector. The results showed that left ventricular ejection fraction, left ventricular end-diastolic pressure, mean pulmonary capillary wedge pressure, myocardial oxygen consumption, left ventricular end-diastolic volume, left ventricular ejection resistance and modulus of arterial elasticity were higher in patients with EH than in the controls. The myocardial ischemic threshold was lower than in the normal subjects (P<0.01). As mentioned above, the left ventricular diastolic disturbance(LVDD)could be a main early signin EH with asymptomatic heart failure. The severity of LVDD in EH had no significant relation with the course of illness and patients ages. It is suggested that in EH patients around 60 years, the heart rate of 55～ 69 beats/min seems beneficial forprevention of myocardial ischemia. This method is more specific, most sensitive and can be useful for early diagnosis, revention and treatmentin EH with asymptomatic heart failure.
