摘要:
为了对严重烧伤后心泵功能的降低进行有效的防治, 本文选用Wistar大鼠24只, 观察1,6-二磷酸果糖(FDP) 对烧伤大鼠离体心脏功能和血液流变特性的影响。结果显示, 应用FDP后使血液流变学各参数: 全血粘度、血浆粘度、红细胞压积、纤维蛋白原、血清总胆固醇、甘油三酯、以及红细胞膜的刚度指数、红细胞聚集指数均明显降低, 而氧释放系数显著升高, 与烧伤组相比差别显著, 与对照组相比无明显差别。同时心脏功能的各项指标均由烧伤后的降低而逐渐恢复: 心输出量、左心室内压及其变化速率、冠脉流量、主动脉压力均显著升高, 与烧伤组相比有明显差别, 与对照组相比差别不显著。提示FDP通过改善血液流变特性, 有效地恢复全身的机能状态和微循环功能, 并增强红细胞的运氧能力。FDP还能促进ATP的生成, 增加心肌细胞的能量供应, 从而提高心肌收缩力, 增加心输出量, 使受伤后的心肌维持较好的血液流变学特性。FDP能改善烧伤后的血液特性是烧伤休克早期心功能得以恢复的重要原因之一, 从而为临床烧伤病人的抗休克治疗提供理论依据。
Abstract:
The effects of fructose-1, 6-diphosphate (FDP) on the hemorheology and the function of isolated perfused working heart were studied in 24 rats after burn. The results showed that: blood viscosity, plasma viscosity, hematocrit fibrinogen, cholesterol, triglycerine, aggregation index of RBC were significantly depressed, and the deformation index of RBC was also recovered markedly. However, oxygen release coefficient was elevated (in FDP treated group),as compared with burn group (P<0.05) and with control group (P>0.05).At the same time, heart rate, cardiac output, left intraventricular pressure.±dp/dtmax , coronary flow and aortic pressure were elevated significantly (in FDP treated group ),as compared with burn group (P<0.05),as compared with conrrol group(P>0.05).The results indicated that: FDP improved the hemorheology parameters and hence recovered all body and microcirculation function, and improved the function of oxygen transport of RRBC and accelerated the synthesis of ATP.Thereby, it increased the energy of myocardial cells and mmyocardial contractability and cardiac output.Thus, improvement of the hemorheology parameters by FDP may be one of the important causes for the recovery of cardiac function after burn.
