应用脉冲多普勒超声测定25例先天性心脏病患儿的肺动脉血流频谱参数并与心导管测定的右心室dp/dtmax进行对比分析。结果显示: 肺动脉血流频谱参数(动能最大变化率, dk/dtmax) 与右心室dp/dtmax呈良好相关(r=0.926, P<0.01),直线回归方程:dp/dtmax=0.59dk/dtmax+11.2 (kPa/s) ; 平均加速度mA及p2/AT与p/dtmax亦有较好的相关性。表明由多普勒超声获得的肺动脉血流频谱参数dk/dtmax能较准确地估测右室p/dtmax, 因而可作为评价心肌收缩功能的指标。
Abstract:
In 25 patients with congenital heart defect, pulmonic flow indices measured by pulse wave doppler echocardiography were compared with right ventricular dp/dtmax obtained by cardiac catheterization. The results showed that there was a good correlation between pulmonic flow index (peak rate of kinetic energy change, dk/dtmax) and right ventricular dp/dtmax. The linear regression equation was as follows: dp/dtmax= 0.59 dk/dtmax+11.2 (kPa/s) (r=0.926,P<0.01). A good correlation of dp/dtmax with mA and Vmaxp2/AT was also found. The results suggested that doppler-derived pulmonic index dk/dtmax could be used to estimate RV dp/dtmax. Thus, dk/dtmax may be a reliable index of contractile function.
