摘要:
为评价超声背向散射参数判定心肌组织纤维化的临床价值。采用HP SONOS 2500 型超声检查仪观测24例陈旧性心肌梗死(OMI)患者梗死灶、38例扩张型心肌病(DCM)和45例Ⅱ型糖尿病(DM)患者、正常对照组40例室间隔、左室后壁心肌的背向散射参数——背向散射积分(IBS)和IBS的心动周期变化幅度(CVIB)。结果:OMI梗死灶、DCM 和Ⅱ型DM患者心肌的校正IBS (IB%)平均值均显著增大, CVIB均降低或出现IBS曲线形态明显异常。IB%、CVIB对OM I、DCM、DM均有较高的阳性预告值和阴性预告值。结论: 背向散射参数对判定心肌病变、判断心肌纤维化程度较为客观、简便, 有较高的临床应用价值。
Abstract:
We sought to assess the value of ultrasonic backscatter parameters in the determination of myocardial fibrosis. Myocardial ultrasonic backscatter parameters of patients with old myocardial infarction (OMI, 24 cases),dilated cardiomyopathy(DCM,38 cases),diabetes mellitus (DM , 45 cases) and 40 members of normal control group were measured by a commercially available modified real-time backscatter signal 2D imaging system(HP SONOS 2500).Results: Time-averaged values of corrected myocardial IBS(IB%) of previously infarcted myocardium foci and IVS or LVPW of patients with DCM or DM were all significantly greater than those of the normal. For most patients, patterns of time-IBS curve turned out to be abnormal and CVIB values decreased greatly. IB% , CVIB brought about high positive or negative predictive values to OMI, DCM and DM. Conclusion: Ultrasonic backscatter was useful in determining myocardial abnormality and assessing the degree of myocardial fibrosis. It is an objective and direct technique for clinical application.
