摘要:
目的 用心率变异性(HRV)观察肾脏病及肾功不全患者变化。方法 选择肾功正常的肾病患者26例,肾功不全患者34例,正常对照组34例。用24 h动态心电图分析其HRV的时域指标。结果 肾功正常肾病组平均RR间距期(RR I)、24 h窦性R-R间期标准差(SD)、5 min窦性R-R间期标准差(SDAIDX)显著低于正常组(P＜0.05);肾功不全组6项指标均明显低于正常对照组,SD,5 min窦性R-R间期标准差的均值(SDIDX),SDAIDX,及R-R连续差异均方根(rMSSD)显著低于肾功正常组。结论 肾脏病患者在肾功正常期即有自主神经功能损伤,以交感神经活动受损主为;肾功不全患者交感神经及迷走神经功能均受损。
Abstract:
Objective To analyze heart rate variability (HRV) in patients with renal diseases or renal failure. Methods Time domain indices of HRV were analyzed using 24-hour ambulatory electrocardiogram recordings. 24 patients with renal diseases and normal renal function (NRF group),34 with renal failure (RF group) and 42 healthy subjects (HS group) were included. Results RRI (RR interval),SD(standard deviation of all normal RR intervals),SDAIDX (standard deviation of the average normal RR intervals for 5-minute segments) in the NRF group were markedly lower than those in the HS group. All of the six HRV indices in the RF group were significantly lower than those in the HS group. SD,SDIDX (mean of standard deviation of all normal RR intervals for 5-minute segments), SDAIDX and rMSSD (root mean square succesive difference) were markedly reduced as compared to with those of the NRF group. Conclusion Autonomic (mainly sympathetic) dysfunction appeared in patients with renal diseases even at normal renal function stage and worsen in patients with renal failure (both sympathetic and parasympathetic dysfuction).
