目的 研究肝素涂层支架植入术对冠状及体循环中血浆内皮素(ET)和丙二醛(MDA)水平的影响。方法 选择冠心病患者30 例,随机植入普通无涂层支架(NCS)16例和肝素涂层支架(HCS)14例,于手术前后不同时间点,同时采集冠状静脉窦和外周肘静脉血检测血浆ET-1和MDA浓度。结果 ①NCS组 ET-1水平在支架后即刻和支架后12 h呈现大小两个高峰,MDA水平在支架后5 min达高峰(P<0.01);②HCS组 ET-1水平在支架后5 min,10 min和12 h明显低于同时间点NCS组水平(P值分别<0.05,<0.01和<0.05),MDA水平于支架后5min至术后1 d均明显低于NCS组(均P<0.05)。 结论 HCS可降低术后ET-1和MDA水平,这可能对支架植入术后防止急性血管痉挛和(亚)急性血栓形成的发生有一定保护作用。
Abstract:
The purpose of our study was to investigate the effects of heparin-coated Wiktor-I stent on the concentrations of plasma endothelin-1(ET-1) and malondialdehyde (MDA). Non-coated stent(NCS)(n=16) and heparin-coated stent(HCS) (n=14) were randomly implanted into 30 patients with coronary artery diseases. Circulating ET-1 and MDA levels were measured in both groups of patients at the paired sampling sites: the coronary sinus and peripheral vein. Results were as follows: 1. NCS group: plasma ET-1 concentration exhibited two peaks immediately and 12 h after the implant, MDA level reached the peak 5 min after the implant (P<0.01); 2. HCS group: ET-1 level was decreased dramatically compared with that in NCS group 5 min, 10 min and 12 hours after the implant (P<0.05,P<0.01 and P<0.05, respectively), and the same with MDA level from 5min to 1d after the implant(all P<0.05). It can be concluded that HCS can decrease post-stent ET-1 and MDA level, so it can be inferred that HCS is helpful in preventing post-stent acute vasospasm and (sub)acute thrombosis.
