目的 初步评价房间隔缺损并发心房纤颤患者应用Amplatzer 伞封堵房间隔缺损后心房纤颤的电复律疗效。方法 2 例患者(51~ 53 岁)。在透视及食管超声心动图监视下经导管置入Amplatzer 伞封堵房间隔缺损。术后即时行超声心动图, 术后24 h, 和1, 3, 6 月, 分别行经胸超声心动图评价房间隔缺损治疗效果。术后6 月行电复律。结果 2 例患者疗效均佳, 房间隔未见残余分流, 电复律成功, 患者转为窦性心律, 复律后无并发症及Amplatzer 伞移位。结论 应用Amplatzer 伞封堵房间隔缺损并发心房纤颤患者, 电复律易成功, 是一种安全有效的复律方法。
Abstract:
AIM To evaluate the efficiency and short term results of electrical cardioversion of atrial fibrillation after transcatheter closure of secundum atrial septal defects (ASD ) using Amplatzer occluder device in ASD patients with atrial fibrillation . METHODS each ASD of two patients (51, 53 years old respectively) was occluded using Amplatzer occluder device through the percutaneous procedure guided by fluoroscopy and transesophageal echocardiography (TEE ).TEE were performed immediately and transthoracic echocardiography (TTE) was done 24h, and 1, 3 and 6mo after occlusion to evaluate the efficiency. Electrical cardioversion were performed to terminate atrial fibrillation 6 mo after the procedure. RESULTS The treatment efficiency of 2 patients was very well. No residual shunt of atrial septal was found in 2 patients immediately after the procedure. There was no complications after electrical cardioversion and sinusrhythm was restored. CONCLUSION Atrial fibrillation could be terminated and sinusrhythm was restored by electrical cardioversion after the occlusion. Electrical cardioversion is safe and effective after occlusion in ASD patients with atrial fibrillation.
