摘要:
目的 评价核潜艇艇员远航前后心功能的变化。方法 艇员49名,采用美国HP1000型彩色超声诊断仪。于胸骨旁左室长轴切面,M型取样线置于2a区,测量左室舒张末内径(LVd)、左室收缩末内径(LVs)及心电图R-R间期,计算左室舒张末容量(ESD)、收缩末容量(ESV)、射血分值(EF)、小轴缩短率(FS)、心输出量(CO)及每搏量(SV);于心尖四腔切面,将M型取样线置于室间隔左室面边缘与左室房室平面(即二尖瓣环)交界处,分别测量舒张早期房室平面位移值(T)及左房收缩引起的房室平面位移值(A)。结果 本组人员远航后血压升高(P<0.05),心率加快(P<0.01)。EDV和SV值增加(108±16vs98±15,P<0.01;69±10vs63±10,P<0.01),主要收缩功能指标包括EF,FS及CO与远航前无明显差异(P>0.05);房室平面位移值A增大(0.64±0.12vs0.56±0.11,P<0.05),T值明显变小(1.34±0.23vs1.49±0.24,P<0.01)。结论 核潜艇远航可引起艇员血压升高,心率加快以及随之而来的心脏舒张功能减低,但收缩功能无明显变化。
Abstract:
AIM To evaluate the cardiac function of nuclear submarine sailors after long sailing. METHODS 49 nuclear submarine sailors were examined by echocardiography. The parameters that were related to the systolic and diastolic cardiac function respectively from LV diameters and index of the atrial-ventricular plane displacement were measured with HP1000 computed sonography.RESULTS EDV and SV were significantly increased (108±16vs98±15,P<0.01;69±10vs63±10,P<0.01) after long sailing of nuclear submarine. The A index was enlarged (0.64±0.12vs0.56±0.11,P<0.01) and T index was significantly reduced (1.34±0.23vs1. 49±0.24,P<0.01).CONCLUSION The diastolic dysfunction and normal systolic function of nuclear submarine sailors ware found after sailing for thirty-one days in this study.
