摘要:
目的 利用Tei index评价经皮腔内冠状动脉成形术(PTCA)对冠心病患者左室功能的影响。方法 冠心病患者60例,分为心绞痛组40例,心肌梗死(心梗组12例),心力衰竭(心衰组8例);正常对照组30例。观察HYNA心功能分级,测定左室舒张末期内径(LVDd),左室收缩末期内径(LVDs),二尖瓣舒张晚、早期血流峰值比值(A/E),二尖瓣舒张早期血流E峰减速时间(EDT),等容舒张期(IRT),左室短轴缩短率(FS),左室射血分数(LVEF),等容收缩期(ICT),射血时间(ET),Tei index。Tei index=(IRT+ICT)/ET,由二尖瓣关闭至开放时间减去ET所得值再除以ET而得。结果 正常对照组Tei index为0.36±0.07,PTCA术前心绞痛组,心梗组,心衰组Tei index分别为0.49±0.15,0.62±0.13, 0.81±0.17,均显著高于正常对照组(P<0.05,P<0.01)。PTCA术后1年,心绞痛组Tei index为0.39±0.08,较术前明显下降(P<0.05),与正常组无显著差异;心梗组和心衰组的Tei index分别为0.47±0.10和0.66±0.14,较术前显著减低(P<0.05),但仍显著高于正常组(P<0.05,P<0.01)。结论 Tei index是评价左室整体功能的简便、准确的多谱勒超声新指标,根据PTCA术前、术后Tei index变化,发现冠心病患者PTCA术后左室功能均有不同程度的改善。
Abstract:
AIM To evaluate the effect of percutaneous transluminal coronary angioplasty(PTCA) on left ventricular function of patient with coronary heart disease by Tei index.METHODS We examined and screened 30 normals and 60 patients with coronary heart disease(CHD),which were divided into angina group (40 cases),MI group (12 cases),CHF group (8 cases), with 2D ultrasound and CDFI. LVDd,LVDs,A/E,EDT,FS,EF,ICT,IRT,ET were measured. Tei index =(ICT+IRT)/ET was obstained.RESULTS Tei index of normal was 0.36±0.07. Before PTCA,Tei index of angina group,MI and CHF group increased significantly than the normal(0.49±0.15,0.62±0.13,0.81±0.17,respectively,P<0.05,P<0.01). After 1 year of the PTCA, Tei index of angina group decreased significantly(0.39±0.08,P<0.05),the index was no significantly different between this group and the normal. Tei index of MI group and CHF group also decreased markedly(0.47±0.10,0.66±0.14,P<0.05),but the mean value of the index was still significantly higher than the normal (P<0.05,P<0.01). CONCLUSION Tei indexis a new, simple and reproducible Doppler index of global left ventricular function. The left ventricular function of patients with coronary heart diseases are improved after PTCA.
