目的:探讨在体情况下心肌跨室壁复极离散度(TDR) 变化对心电图T 波影响的可能机制。方法: 运用单相动作电位(MAP)记录技术,同步记录14只开胸兔的左室心肌心外膜层(Epi),中层(Mid),内膜层(Endo)的MAP,分别予以静脉注射索他洛尔(dl-sotalol),海葵毒素(ATX-II)后,观察跨室壁复极离散的变化及同时心电图T 波的相应改变。结果:①dl-sotalol导致Mid层细胞MAP的复极时间(RT)显著的延长(从202±19 ms到395±34ms),TDR增大(从11±4ms到75±25ms),Q T间期延长(从208±16ms到397±33ms),3层心肌MAP的3相复极不同程度的延长,使复极电位梯度变化,产生增宽、低幅有切迹的T波。②ATX-II 导致M id 层细胞MA P 的RT 显著的延长(从370±34 m s 到473±35 m s) , TDR 增大(从40±21 m s 到62±19 m s) ,Q T 间期延长(从372±33 m s到479±33ms),3层心肌MAP的2相平台期不同程度延长,使复极电位梯度变化,产生晚现T波,波幅增大。结论:在体兔心肌跨壁复极离散度的变化对心电图T波的形态有重要影响。
Abstract:
AIM: To investigate the variation in transmural dispersion of repolarization and its effect on T-wave in ECG in vivo. METHODS: With the monophasic action potential (MAP) recording technique, MAPs of epicardium, midmyocardium and endocardium were recorded by specially designed plung-needle elect rodes at the left ventricular myocardium in 14 open-chest rabbits. After in travenous inject ion of dl-sotalol and A TX-II respectively, variation in transmural dispersion of repolarization on three myocardial layers and T-wave in ECG were investigated. RESULTS: ①dl-sotalo l caused a preferential prolongation of the Mid-cell repolarization time (RT , 202±19ms to 395±34ms), giving rise to broad and some times low amplitiude bifurcated T waves, and increased TDR (11±4 ms to 75±25 ms) and Q T in terval (208±16 ms to 397±33 ms) ; ②A TX-II caused preferential prolongation of the Mid-cell RT (370±34 ms to 395±34 ms) , a marked delay in the onset of the T waves, a sharp rise in TDR (40±21 ms to 62±19 ms) and Q T in terval 372±33 ms to 479±33 ms). CONCLUSION: The variation in transmural dispersion of repo arization of vent ricular myocardium has important effects on the T wave of ECG in vivo of the rabbit.
