摘要:
目的:考察低分子量肝素(LMWH)对高龄不稳定型心绞痛(UAP)患者凝血、纤溶和抗凝指标的影响。方法:UAP患者40例,年龄75~86岁。给予LMWH (Fraciparine, 商品名速避凝) 0.4 ml,腹壁皮下注射,每12h1次,疗程7d。于用药前、末次用药后4h和12h分别抽血测定抗Xa因子活性、血浆凝血酶原时间(PT)、活化部分凝血活酶时间(aPTT)、血浆D-二聚体(D-Dimor)、血浆组织型纤溶酶原激活物(tPA)活性、血浆纤溶酶原激活物抑制物(PAI)及抗凝血酶Ⅲ(AT-Ⅲ)活性。结果:用药后PT、aPTT轻度延长;血浆抗Xa活性及tPA水平增高(P<0.05);D-Dimer含量在LMWH注射后显著降低(P<0.05)。结论:LMWH治疗高龄UAP,皮下注射,每12h1次,可以获得持续的抗血栓效果,可考虑不需要实验室监测。
Abstract:
AIM:To assess the influence on hemostasis, fibrinolysis and anticoa gulation systoms of low molecular weight heparin(LMWH), intreatment of unstable angina pectoris(UAP). METHODS: 40 patients aged 80 years with UAP were treated with LMWH (fraxiparine) ubcutaneous in injection once every 12h for 7days. The plasma PT,aPTT,AXa,D-Dimer, tPA, PAI and AT-Ⅲ were detected before thet reatment and 4h,12h after the last single subcutaneous injection. RESULTS: The laboratory resultss howed that PT,aPTT prolonged slightly (P<0.05), AXa,tPA levels increased (P<0.05) and D-Dimeractivity decteased (P<0.05) after treatment. CONCLUSION: LMWH can be used in very elderly patients with UAP effectively and do not require laboratory determination.
