摘要:
目的:探讨C型钠尿肚(CNP)对妊商征(PIH)患者的心功能效应及与一氧化氮(NO)水平的相关性。方法:重度PIH患者40例随机分为CNP组及安慰剂对照组,每组各20例。分别给予CNP( 6 ug/kg)或安慰剂静滴30 min,应用美国Acuson 128xP/10型彩色电脑声像仪,监测用药前后心功能参数:每搏输出量 (SV),心排出量(CO),左室射血分数(LVEF),心脏指数(CI),心率(HR)及血压(BP)的变化,并测定静脉血NO代谢终产物亚硝酸基/硝酸基 (NO2一/NO3一)水平.取正常孕妇40例为血流动力学基础值对照组.结果:PIH组与对照组比较,血流动力学参数有增高、降低或正常,各参数波动范围明显增加,但CO及HR均值明显增高(P<0.05). CNP治疗30 min后平均动脉压(MAP)下降(P<0.01 ).SV,CO,CI及HR均显著下降(P< 0.05 ),各参数下降值与基础值呈显著正相关(P<0.01 ),NO2-/NO3一水平显著增高(P<0.01 ),但CO及HR均无显著相关性(P>0.05 )。安慰剂组各参数及NO2一/NO3一水平变化差异均无显著意义(P>0.05 ),结论:CNP对PIH患者的心功能高动力学状态具有降低效应并增加NO产生,但NO可能不直接参与CNP对PIH患者的心功能调节。
Abstract:
AIM:To investigate the effects of C-type natriuretic peptide (CNP) on the cardiac function and their correlation with the levels of serum nitric oxide (NO) in patients with pregnancy induced hypertension (PIH). METHODS:40 severe PIH patients were divided randomly to two groups and infused intravenously with CNP (6 ug/kg) or placebos for 30 minutes. Parameters of cardiac function in terms of strode volume (SV),cardiac output (CO),LV-ejection fraction (LVEF),cardiac index (CI) and heart rate (HR) were detected with Acuson 128 XP/10 computed sonography and blood pressure (BP)was recorded and serum nitrite/nitrate (NO2一/NO3一),the end products of NO were measured before and after the infusion of either CNP or placebos. 40 normal pregnancy women served as the control group of basic parameters. RESULTS:Compared with control group,parameters of cardiac function in PIH patients were increased, decreased or normal and the parameter ranges were obviously extended. However the meanc ardiaco utput( CO)a ndh eartr ate( HR)w eres ignificantlyi ncreased( P<0.05 r espectively).After the infusion of CNP,the mean arterial pressure (MAP) was significantly decreased (P <0. 01). HR,SV, CO and CI were also significantly decreased (P<0. 05,respectively) and there was a positive correlation between each decreased index and each basic index( P<0.O 1,respectively).The level of serum NO2-/NO3- was significantly increased( P<0.0 1).In the placebos group,there were no significant changes in cardiac function parameters,MAP and the level of serum NO2一/NO3一.No significant correlations were observed between the level of serum NO2一/N03一,and CO and HR (P > 0. 05). CONCLUSION:CNP inhibited the high cardiac function and enhanced NO production,but NO may not be involved directly in the process of function and enhanced NO production,but NO may not be involved directly in the process of CNP regulating the cardiac function in PIH patients.
