摘要:
本文报告了我院1994～2003 年收治的20 例主动脉夹层(AD) 患者。对AD 的诊断和治疗有如下体会:胸片可提示诊断;超声检查简便易行、敏感性和特异性均高;CT、MRI、动脉造影均是确诊本病的重要影像检查手段,主要阳性表现有双腔征、内膜撕裂口、内膜飘浮片等,螺旋CT动脉造影三维重建(3D-CTA) 对诊断本病有独特价值,可非常清晰地显示内膜的原发撕裂口和继发撕裂口、夹层的部位、范围、走行、主动脉分支的受累情况,价值比MRI 更高。内科保守治疗可为外科手术和(或) 介入治疗赢得时间,抗血小板和抗凝治疗可增加早期病死率,应慎用或禁用。
Abstract:
This paper reports on the treatment of 20 cases of Aortic Dissection (AD) sufferers hospitalized between 1994 and 2003. X-ray thorax image may give hints. Ultrasonic scanning can boast its convenience as well as its sensitivity and distinctiveness. CT , MRI , radiography can also provide evidence for diagnosing. The major positive signs include double cavity sign , endocardium cleft and floats , etc. A 3-D reconstructed spiral CT arterial radiography (3-D CTA) is so valuable that it can form a clear picture of the primary and the subsequent endarterium cleft , the position of aortic dissection as well as the scope and tracking , and also the extent of aorta ramification involvement . Conservative medical therapy may save time for surgicals and/ or interposition treatment . However , as anti-blood platelet therapy in addition to anti-agglutinate therapy may increase the rate of earlier deaths , it is , therefore , advised that this therapy be cautiously adopted or even forbidden in treatment .
