目的:探讨犬肺静脉(PV) 、心房电生理特性随年龄变化特点。方法:健康杂种犬15 只按年龄分成3 个阶段组:老、中、幼龄组,每组5 只。经锁骨下、股静脉穿刺和房间隔穿刺放置导管,采用刺激诱发心房纤颤(AF) ,标测肺静脉PV、上腔静脉(SVC) 、左、右心房电位,测量各自有效不应期( ERP) 、功能不应期( FRP) ,传导时间。结果:3 组AF 诱发率分别为幼龄组0 %、中龄组20 %、老龄组100 %;起源于PV 的AF 占83. 3 % ,其它为16. 7 %; ERP :老龄犬PV 的ERP 显著短于幼龄组,为132 ±38 ms vs 234 ±51 ms( P < 0. 01) ,老龄组的PV 的ERP 显著短于心房的ERP ,为132 ±38 ms vs 200 ±34 ms( P < 0. 05) ;老龄犬组SVC 的ERP 显著短于幼龄组( P < 0. 05) 。老、中、幼3 组PV 起搏部位发生递减性传导百分率分别为89 %、33 %、31 % ,传导时间延长分别为92 ±55 ms、27 ±31 ms、22 ±38 ms ,老龄组与后两者比较均有非常显著差异( P < 0. 01) 。结论: PV 电生理特点随年龄变化, 电重构集中发生在老龄阶段,主要表现为不应期显著缩短和递减性传导,可能是PV 致AF 的主要原因。
Abstract:
AIM: To investigate the variations in at rium and pulmonary veins with aging. METHODS :15 dogs were allocated to three groups : the old , the middle aged , and the pup , with five dogs in each group. Catheters were int roduced percutaneously through the femoral and jugular veins in all the 15 dogs. A t ransseptal catheterization was performed under fluoroscopic guidance for int roducing circum-ferential mapping and stimulating catheters dedicated to PV mapping. The ERP ,FRP ,and duration of conduction was determined at each elect rode site by the ext rastimulus technique with a programmable stimulator . RESULTS :The rates of AF occurrence were 0 %for the pup , 20 %for the middle aged and 100 %for the old , with 83. 3 % resulting f rom PV and 16. 7 % f rom other sources. The ERP of PV was 132 ±38 ms in the old dogs and 234 ±51 ms in the pup dogs ( P < 0. 01) . In old dogs , the ERP of PV was ignificantly shorter than that of the at rium (132 ±38 ms vs 200 ±34 ms , P < 0. 05) , and ERP of SVC was shorter than in pup dogs ( P < 0. 05) . In PV , decremental conduction occurred (89 % in old dogs vs 33 % in middle age dogs , P < 0. 01 ; 89 % in old dogs vs 31 % in the pup , P < 0. 01) ,and increment of conduction time : 92 ±55 ms in old dogs vs 27 ±31 ms or 22 ±38 ms in the other two groups , P < 0. 01. CONCLUSION: Elect ro-remodeling of PV which occurs f requently at old age may be the main source of AF , with the presenation of significantly short ERP and increment of conduction time.
