目的 探讨羟甲基戊二酰辅酶A(HMGCoA)还原酶抑制剂氟伐他汀防治缺血性心脏病心衰疗效及其抗氧化应激作用。方法 雌性SD大鼠42只,急性心肌梗死(AMI)术后6 h随机分为:①AMI对照组(n=24);②氟伐他汀组(n=18);③另设假手术组(n=10)。直接灌胃给药8周后行高频多普勒超声、血流动力学、心脏重塑指标、左心室α、βMHC的mRNA表达、心肌总抗氧化能力(TAOC)和丙二醛(MDA)测定。结果 与假手术组相比,AMI组左室舒张末期内径(LVEDd)、左室舒张末期容积(LVEDV)、E峰、E峰减速度、E/A、左室舒张末压(LVEDP)、左、右心室心肌肥厚指数、βMHCmRNA和MDA均显著增加,左室短轴缩短率(FS)、射血分数(EF)、αMHC mRNA和TAOC均显著降低。与AMI组相比, 氟伐他汀组的LVEDd、 LVEDV、E峰、E峰减速度、E/A、LVEDP和左、右心室心肌肥厚指数、βMHC mRNA及MDA均显著降低,FS、EF、αMHC mRNA和TAOC显著升高。结论 氟伐他汀能抑制心室重塑,预防缺血性心脏病心衰发生;抗氧化应激作用可能是其机制之一。
Abstract:
AIM To study the effect of Fluvastatin on the treatment of heart failure due to ischemic heart disease and on the inhibition of oxidative stress. METHODS Six hours after the ligation of left coronary artery, 42 survived AMI female SD rats were randomly assigned to: The AMI control group(n=24), the fluvastatin group(n=18 ), and the shamoperated group(n=10), selected randomly as the noninfarction control. After 8 weeks of therapy with drugs by gastric gavage, cardiac function was assessed by Doppler echocardiography, hemodynamics, ventricular weight/body weight ratio, noninfarcted myocardial MHCmRNAs, the myocardial concentration of total antioxidation capacity(TAOC) and malondialdehyde(MDA). Exclusive of rats with MI size ﹤30﹪ or ﹥50﹪,complete data were obtained in 33 rats, which comprised the AMI control(n=12), fluvastatin(n=11) and sham operated groups(n=10). RESULTS There was no significant difference in MI size between the two groups (P﹥0.05). Compared with the shamoperated group, left ventricular enddiastolic dimension(LVEDd), left ventricular enddiastolic volume (LVEDV), Ewave, Ewave deceleration, Ewave/Awave velocity ratio, left ventricular enddiastolic pressure(LVEDP), relative weight(LVRW), right ventricular relative weight(RVRW), βMHC mRNA and MDA were all significantly increased, while fractional shortening(FS), ejection fraction (EF),αMHC mRNA and TAOC were all significantly decreased in the AMI group. In comparison with the AMI group, LVEDd, LVEDV, Ewave, Ewave deceleration, E/A, LVEDP, LVRW,RVRW,βMHC mRNA and MDA were all significantly decreased, while FS,EF,αMHC mRNA and TAOC were all significantly increased in the fluvastatin group. CONCLUSION LV remodeling and heart failure due to ischemic heart disease could be attenuated by Fluvastatin. These effects were associated with its antioxidative feature
