摘要:
目的 总结应用新型成角蘑菇伞治疗小儿动脉导管未闭(PDA)的经验,并对其疗效进行初步分析。方法 全组共15(男5,女10)例,年龄5.5个月~8 岁(2.8±2.5岁),3岁以内婴幼儿12例,体质量6~30 kg。PDA最小内径2~12(5.9±3.1) mm,术前均有肺动脉高压,平均肺动脉压为34~65 mm Hg。选用新型封堵伞直径6~20(12.9±4.2) mm。结果 15 例均获得成功。重复主动脉造影,无主动脉腔内狭窄和变形,无明显造影剂分流后释放封堵器。术后12例即刻无分流,3例有少量残余分流,术后24 h 超声心动图示分流消失,肺动脉压术后均有下降。结论 应用新型成角蘑菇伞治疗小儿特殊类型PDA适应证范围广,成功率高,治疗效果好。
Abstract:
AIM To analyze the efficiency of transcatheter closure of patent ductus arteriosus(PDA) using the new angled duct occluder in the children. METHODS Percutaneous closure of PDA was attempted in fifteen cases with a median age of 2.8 years (range, 5.5 months~8 years) and range weight of 6~30 kg. The mean minimum diameter of the PDA was 5.9±3.1 mm (range, 2.0~12 mm). The mean pulmonary pressure ranged 34~65 mm Hg in all cases with pulmonary hypertension. The mean diameter of the devices was 13±4 mm (range, 6~20 mm). RESULTS The device was successfully implanted in the 15 patients. There were no complications. Aorgraphy was performed immediately after the procedures that no sign of the stenosis and distortion of the thoracic aorta. Immediate complete closure was 12 cases and the trivial residual shunt was found in 3 cases. The UCG complete closure was done 24 h after the transcatheter closure. Pulmonary pressure decreased in all cases with pulmonary hypertension. CONCLUSION The new angled duct occluder device was a safe and effective for PDA closure, particularly suitable for infants and patients with small PDA ampulla.
