摘要:
目的 研究在小儿胸腔镜引导纤维软镜下,右室流出道畸形矫治的动物手术方法。方法 选用心肺功能良好的杂种犬各30只,分别为实验组和对照组,其中实验组按手术前后顺序又分为A、B、C 3组。实验组进行胸腔镜引导纤维软镜体外循环下模拟右室流出道畸形解除手术;对照组进行胸腔镜体外循环下房间隔缺损修补手术。分别记录两组的平均动脉压、中心静脉压和心率;实验组还记录手术时间、主动脉阻闭时间和转机时间。结果 手术过程中,实验组与对照组不同时段的平均动脉压、中心静脉压和心率均无显著差异。经过练习后,与A组相比,C组手术时间、主动脉阻闭时间和转机时间均显著缩短(P<0.05)。结论 胸腔镜引导纤维软镜下进行右室流出道的动物手术是安全可行的,手术时间与镜下操作的熟练程度有关。
Abstract:
AIM To investigate the operation method for corrective therapy of right ventricle outflow tract deformity under infant fibrothracoscope leading. METHODS Sixty dogs with satisfactory heart and lung functions were employed in this study. In control group (n=30), the repair of atrial septal defect was carried out by extracorporeal circulation under thoracoscope, while in experiment group (n=30), the corrective operation of right ventricle outflow tract deformity was conducted by extracorporeal circulation under fibrothracoscope leading. Experiment group subdivided into three groups, namely A, B and C based on sequence before and after operation. Mean arterial pressure, central venous pressure and heart rate in both groups were recorded respectively. The operating time, aortic blocking time and cardiopulmonary bypass time were also recorded in the experiment group. RESULTS There was no significant difference between experiment group and control group in mean arterial pressure, central venous pressure and heart rate. After practice, the operating time, aortic blocking time and cardiopulmonary bypass time in the group C were notably shorter than those in the group A. CONCLUSION The animal operation of right ventricle outflow tract surgery under fibrothracoscope leading is safe and feasible. Operating time is related to the surgeon′s dexterity under fibrothracoscope.
