目的 通过检测S100蛋白与肿瘤坏死因子-α(TNF-α)和白细胞介素-8(IL-8)的关系,探求冠状动脉旁路移植术(CABG)与全身炎症反应综合征(SIRS)的关系。方法 从术前水平到术后24 h共6个时间点经颈内静脉采集40例患者血标本,测定血浆S100蛋白、TNF-α、IL-8浓度,并对患者术后精神神经系统进行系统检查,评价患者围术期神经系统状况。结果 所有患者术后均无死亡及明显脑损伤。再灌注后5 min时S100蛋白浓度与TNF-α和IL-8相关关系分别是:r=0.83, P<0.01; r=0.76, P<0.05。结论 S100蛋白浓度与TNF-α和IL-8浓度呈正相关,显示血浆S100蛋白浓度的早期升高可能与全身炎症反应和再灌注损伤有关。
Abstract:
AIM To observe the relationship of cerebral injury and systemic inflammatory response syndrome (SIRS) by investigating the relationship between S100 protein and TNF-α and IL-8. METHODS S100, a protein specific for cerebral tissue, was used as a marker for cerebral impairment and 40 Patients who had CABG from October in 2002 to January in 2003 were studied. The blood samples were taken from internal jugular vein at 6 time points from 24 hours preoperation to 24 hours after the cardiac surgery. S100 levels and TNF-αand IL-8 of both groups were measured. We also evaluated perioperational state of the nervous system by examining the patients′ neuropsychological system. RESULTS There were no deaths or major neurologic complications in all the patients. S100 levels at 5 min after reperfusion had significant correlation with TNF-α and IL-8 (r=0.83, P<0.01; r=0.76, P<0.05). CONCLUSION The positive correlation of S100 levels with TNF-α and IL-8 suggests that SIRS due to CABG may play an important role in the sharp rise of S100 concentration.
