迷走神经和乙酰胆碱对缺血性心脏病的可能抗炎保护作用(PDF)

摘要:
缺血性心脏病(ischemic heart diseases,IHD)是由于心脏冠脉循环改变,引起冠脉血流和心肌需求之间不平衡而导致的心肌损害。随着对IHD发病机制的深入研究,发现迷走张力的低下可作为独立因素诱发IHD;而缺血发生后心肌产生的大量细胞因子又可进一步损伤缺血心肌的功能。近年来,许多研究对内毒素和脂多糖诱导的炎症、全身性免疫炎性疾病、缺血/再灌注损伤以及神经元损伤等实验模型的研究表明,刺激迷走神经或增加乙酰胆碱(ACh)可通过活化α7烟碱型乙酰胆碱受体,抑制多种细胞因子的产生从而发挥保护作用。在心肌梗死后慢性心力衰竭的动物模型中,刺激迷走神经可显著抑制严重心律失常的发生率和致死率、增加长期存活率。本文拟通过对迷走神经及其递质ACh的抗炎作用以及IHD发生前后迷走张力降低的特征阐述改善迷走张力或增加ACh对缺血心肌潜在的抗炎保护通路。这一通路可能是经过α7nAChR激活后续的下游分子从而抑制多种细胞因子的产生。
