目的 观察低强度体外治疗性超声对冠心病患者血浆组织型纤溶酶原激活剂(tPA)、纤溶酶原激活剂抑制剂1(PAI1)的影响,评价超声对纤溶系统的作用,初步探讨超声助溶的机制。方法 选择拒绝接受介入性治疗的冠心病(包括稳定型心绞痛和不稳定型心绞痛)患者70例,随机分为超声治疗组(39例)、超声对照组(31例)。超声治疗组患者分别给予1.2 W/cm2的超声治疗,每天1次,每次15 min。两组患者分别于观察治疗前和观察治疗后10 d后采血,测定血浆tPA、PAI1水平。结果 治疗前,超声对照组与超声治疗组tPA、PAI1、tPA/PAI1均无显著差异。治疗后,超声治疗组tPA显著高于超声对照组,超声对照组与超声治疗组治疗后PAI1无显著差异。结论 体外治疗性超声作用于冠心病患者的心前区,可使血浆tPA水平显著升高。
Abstract:
AIM To observe the effects of lowdose external therapeutic ultrasound on plasma tissuetype plasminogen activator (tPA) and plasminogen activator inhibitor1 (PAI1) in coronary heart disease patients, to evaluate the effects of external therapeutic ultrasound on activity of fibrinolytic system and to explore the mechanism of ultrasound in promoting thrombolysis function. METHODS Seventy patients with coronary heart disease (including the stable angina and unstable angina pectoris) were selected, who refused the interventional therapy and were randomly divided into ultrasonic therapy group (39 patients) and ultrasonic control group (31 patients). Patients in ultrasonic therapy group received the ultrasound therapy of 12 W/cm2 once a day and 15min every time. Blood was collected and plasma tPA and PAI1 were determined separately in three groups before and ten days after therapy. RESULTS Before therapy, the indexes of tPA, PAI1 and tPA/PAI1 had no difference between ultrasonic therapy group and ultrasonic control group. After therapy, plasma tPA in ultrasonic therapy group was much higher than that in ultrasonic control group. tPA/PAI1 in ultrasonic therapy group was also higher than that in ultrasonic control group. No difference of PAI1was seen between the two groups. CONCLUSION External therapeutic ultrasound notably increases the plasma tPA activation, when it acts on the precordial region in patients with coronary heart disease.
