目的 探讨梗死前心绞痛(PA)不同病程时间对ST段抬高心肌梗死(STEMI)近、远期预后的影响并分析预后相关因素。方法 随访2001年1月~2006年1月住院的326例首次STEMI患者,随访到300例,根据PA病程时间分为长期(A)、近期(B)、短期(C)3组,比较临床特征及主要不良心脏事件(MACE)发生率。结果 ①A组总心源性死亡率及远期主要不良心脏事件(MACE)发生率显著高于C组。②COX分析显示首次心肌梗死后未行PCI是STEMI患者死亡的独立预测因素。结论 长期心绞痛患者STEMI后远期MACE发生率高于短期心绞痛组;首次心梗后未行PCI是STEMI患者死亡的独立预测因素。
Abstract:
AIM To retrospectively evaluate the effects of different course of preinfarction angina (PA) on shortterm and longterm prognosis in patients after first ST segmentelevated myocardial infarction (STEMI) and to analyze the affecting factors. METHODS Of the 326 consecutive patients with first STEMI hospitalized in our hospital from January of 2001 to January of 2006, 300 patients were followed up and assigned respectively to longterm group (A), mediumterm group (B), and shortterm PA group (C) according to different course of PA. The clinical characteristics and incidence of major adverse cardiac events (MACE) were compared retrospectively. RESULTS In group A, the total cardiogenic mortality and incidence of MACE were higher than those in group C (P<001). Cox regression analysis showed that no treatment of PCI after first STEMI was an independent predictor of death. CONCLUSION In the longterm PA group, the incidence of MACE is higher than that in the shortterm PA group. No treatment of PCI after first STEMI is an independent predictor of death.
