目的: 总结先天性主动脉窦瘤临床治疗经验，探讨手术要点。 方法: 对我院1997年11月~2007年12月 30例先天性主动脉窦瘤手术治疗进行回顾分析。 结果: 1例行窦瘤切除并补片修补+室间隔缺损补片修补+主动脉瓣置换术患者，术后3 d时突发室性颤动死亡，1例患者主动脉瓣成形术后残余轻度主动脉瓣关闭不全，随访3年时，渐加重为中度主动脉瓣关闭不全，心功能（NYHA）Ⅲ级，失访1例，其余27例患者随访2个月~10年，心功能Ⅰ级20例，Ⅱ级7例。结论: 对窦瘤破裂者，应积极手术治疗，以免延误手术时机，并发病变应同时矫治，合理的处理主动脉瓣关闭不全有利于患者长期预后。
Abstract:
AIM: To review the clinical experience in the treatment of congential aneurysm of sinus of Valsalva and to investigate the surgical focal point. METHODS: Thirty patients with congenital aneurysm of sinus of Valsalva were operated on from November 1997 to December 2007. RESULTS: One patient, undergoing repair of aneurysm and ventricular septal defect and aortic valve replacement, died unexpectedly from ventricular fibrillation at the third postoperative day. One patient, undergoing aortic valvuloplasty, had mild aortic valve insufficiency postoperatively and developed into medium aortic valve insufficiency 3 years later. Cardiac function is currently NYHA class III. One case was lost to follow-up. The rest of the 27 patients were followed-up from 2 months to 10 years. Cardiac function of 20 patients was reported as NYHA class I and seven patients were reported as class II. CONCLUSION: Surgery for aortic sinus aneurysm should be performed as early as possible. Associated abnormalities must be corrected. Long-term results are associated with proper treatment of aortic valve regurgitation.
