目的: 总结Hybrid手术镶嵌治疗成人右室双出口并发右室流出道狭窄的疗效。方法: 对21例成人右室双出口右室流出道狭窄并发侧支血管患者进行根治手术和侧支血管封堵手术。将患者分成两组，一组为侧支血管封堵后行外科根治手术，另一组为行外科根治手术后进行侧支血管封堵术。结果: 12例患者在根治手术前行侧支血管封堵术，所有患者术后恢复顺利，痊愈出院，1例患者术后出现严重咯血再次行支气管动脉封堵术。9例在根治术后行侧支封堵术，术后患者呼吸机使用时间长，住ICU时间长，死亡1例。结论: 外科手术前封堵侧支血管较术后封堵效果好。
Abstract:
AIM: To explore the clinical effect of hybrid therapy performed in adults with double output of right ventricle (DORV). METHODS: Transcatheter closure of collateral arteries was performed in 12 cases with huge aorta pulmonary collateral arteries (APCAs) before total correction of tetralogy of Fallot (TOF) and in nine cases after total correction. RESULTS: In the 12 cases of transcatheter closure before correction, pulmonary blood volume decreased and satisfactory results were achieved. In the nine cases of transcatheter closure after correction, one patient died and the time necessary for mechanical ventilation and ICU were longer. CONCLUSION: Hybrid therapy for DORV with huge APCAs is safe, simple and effective. Transcatheter closure before total correction achieves better results than transcatheter closure after total correction.
