目的:通过采用社区医院一体化管理模式，对老年慢性心力衰竭（CHF）患者进行院内、院外连续性全程管理，以此来探索一种合理的CHF社区管理模式，以提高CHF患者的生存质量，降低医疗费用。方法： 将社区内普查出的CHF患者按居住区域分为干预组、对照组。对照组进行传统药物治疗，干预组在传统药物治疗的基础上再施加行为干预措施。比较两组患者经过两年不同的处理方法后，各项观察指标的差异。结果： 干预组的再入院率、再入院次数和医疗费均显著低于对照组（均P<0.01），干预组两年存活率、6分钟步行距离显著高于对照组(P<0.01)。结论： 社区医院一体化管理可以明显提高老年CHF患者的治疗效果，并可以有效的降低医疗费用，减少再住院率。
Abstract:
AIM:To explore the social effect of hospital and community-integrated management for elderly patients with chronic heart failure. METHODS: Patients with chronic heart failure in the community were divided into experimental group of conventional drug treatment and control group of conventional drug treatment plus behavioral intervention. The results of the two groups were compared after 2 years of treatment. RESULTS: The prognosis-associated indexes in the experimental group were better than those in the control group. CONCLUSION: Hospital and community-integrated management significantly improves treatment results for elderly patients with chronic heart failure and effectively reduces medical costs and rate of re-hospitalization.
