摘要:
目的:探讨同型半胱氨酸(Hcy)、血清胱抑素C(Cys-C)与高血压早期肾损害的相关性。方法:收集我院2011年6月~2013年7月符合入选标准的153例高血压病患者,用同位素肾图检查作为金标准测定肾小球滤过率(GFR),按照GFR数值分成两组:肾功能正常组和肾功能受损组。同时测定每组血清Hcy、Cys-C和24 h尿微量白蛋白(24h MAU)、血肌酐及尿素氮,比较两组之间是否有差异,最后对GFR与Cys-C及Hcy进行相关性分析。结果:肾功能无损害组与肾功能损害组Hcy、Cys-C比较差异有统计学意义(P<0.05,P<0.01)。Pearson相关分析显示,GFR与Hcy、Cys-C和尿素氮及肌酐呈负相关(均P<0.01)。多元线性回归分析显示:Hcy、Cys-C是影响GFR的独立危险因素,其标准偏回归系数β值分别为-2.015(P<0.05)、-5.448(P<0.01)。结论:Hcy、Cys-C与原发性高血压早期肾损害有相关性。
Abstract:
AIM: To investigate the relationship between homocysteine, serum cystatin and early renal impairment in patients with hypertension. METHODS: One hundred and fifty-three patients from our hospital meeting the inclusion criteria from June 2011 to July 2013 were included in the study. Isotope renogram was performed to measure the kidney glomerular filtration rate (GFR). Patients were divided into two groups according to GFR results: nonimpaired renal function group and impaired renal function group. Serum Cystatin-C, 24h MAU, creatinine (SCr) and blood urea nitrogen (BUN) were also measured and the correlation between GFR and Hcy, Cys-C was analyzed. RESULTS: Statistical differences were found in homocysteine and Cys-C between groups and Pearson correlation analysis showed that GFR was negatively correlated with homocysteine, Cys-C, SCr and BUN (all P<0.01). Multiple regression analysis showed that homocysteine and Cys-C were independent risk factors for GFR with partial regression coefficient β, respectively, -2.015 (P<0.05) and -5.448 (P<0.01). CONCLUSIONS: Homocysteine and Cys-C are closely correlated with early renal impairment in essential hypertension.
