摘要:
目的:观察社区-医院一体化管理模式对老年慢性心力衰竭（CHF）患者及家属健康教育达标率的影响和对患者生活质量的改善情况。方法： 将社区内普查出的老年CHF患者按居住区域分为干预组和对照组，比较两组患者经过两年不同的处理方法后各项观察指标的差异。结果： 干预组患者经过2年的社区-医院一体化管理模式的干预，在服药依从性、患者及家属的健康教育达标率、生活质量等方面，均优于单纯药物治疗的对照组患者，统计学差异显著（P<0.01），干预组再住院率显著低于对照组（27% vs. 76%，P<0.01）。结论： 社区-医院一体化管理可以明显提高老年CHF患者的服药依从性、改善生活质量，提高患者及家属的健康教育达标率。
Abstract:
AIM:To explore the effect of community and hospital seamless management on quality of life and family health education in elderly patients with chronic heart failure (CHF). METHODS: Elderly patients with CHF in the community were divided into two groups according to residential areas: experimental group of community and hospital seamless management and control group of only drug treatment. Differences between groups were compared after 2 years. RESULTS: Medication compliance, quality of life and family health education in experiment group were significantly better than those in the control group (P<0.01). CONCLUSION: Community and hospital seamless management markedly improves medication compliance, quality of life and family health education in elderly patients with CHF.
