目的 分析侧壁QRS波切迹(nQRS)与心脏再同步化治疗(CRT)关联性。方法 将接受CRT治疗的慢性心力衰竭患者41例,按术后有无反应分为有反应组(n=28)和无反应组(n=13),分析两组基线资料有无显著差异(单因素分析),再进行多因素Logistic回归分析。结果 共纳入41例患者,男性28例(68%),平均年龄59岁。经单因素分析发现,有反应组与无反应组出现侧壁nQRS的病例数分别为19例(68%)、4例(31%),两组间差异有统计学意义(P<0.05);QRS波群时限(QRSd)相比无反应组比有反应组更宽(P<0.05)。经多因素logistic回归分析侧壁nQRS〔OR=4.750,95%CI(1.148,19.650),P<0.05〕与CRT术后无反应相关。结论 侧壁nQRS是CRT术后有无反应的独立相关因素。
Abstract:
AIM To assess the association between nQRS in lateral leads and response to cardiac resynchronization therapy (CRT). METHODS 41 heart failure patients after CRT were divided into responder (n=28) and non-responder (n=13), The differences were analyzed in baseline data between the two groups (single factor analysis), and then a multiple factor logistic regression analysis was utilized. RESULTSThere were 41 patients selected, 28 patients were male with an average age of 59 years. After univariate analysis, nQRS in lateral leads were seen in 19 (68%) patients in the responder group compared with only 4 (31%) patients in the non-responder group (P<0.05). QRSd of the responder group was wider than the non-responder group (P<0.05). nQRS in the lateral leads before CRT [OR=4.750, 95%CI(1.148, 19.650), P<0.05] was significant according to logistic regression analysis. CONCLUSION nQRS in the lateral leads was observed to be an important correlative factor for response to CRT.
