目的 研究心肌超声组织背向散射积分(integrated backscatter,IB)心动周期平均变动量(average cardiac cyclic variation of IB,ACVIB)与心脏舒张功能的关系,探讨应用ACVIB评估心脏舒张功能的可行性。方法 按WHO/ISFC-1995制定的标准诊断为肥厚性心肌病(HCM)的患者18例定为HCM组,健康人22例定为对照组。应用Agilent-5500型超声诊断仪,①检测2组研究对象的左心室(LV)IB曲线,记录LV乳头肌水平和腱索水平2个短轴切面的左室前间隔、后壁心肌组织的IB心动周期变动量(cyclic variation of IB,CVIB),计算出ACVIB;②检测2组研究对象的LV舒张功能。③对40例研究对象的ACVIB与LV舒张功能各指标的关系进行直线相关回归分析。结果 HCM组与对照组比较:ACVIB显著减低,同时心脏舒张功能显著减退。相关回归分析结果表明,40例研究对象LV心肌ACVIB的减小与大部分心脏舒张功能指标变化密切相关,包括:①与Ve和Ve/Va相关(r=0.58和0.65,均P<0.01;②与DT相关(r=-0.83,P<0.01);③与Vd以及Vs/Vd密切相关(r=-0.57及-0.68,均P<0.01)。结论 HCM患者的LV心肌ACVIB的改变与LV舒张功能减退之间存在变化规律,ACVIB的减小与舒张功能减退密切相关。通过观察心肌ACVIB的改变,可评估HCM患者心脏舒张功能的变化。
Abstract:
AIM To study of the relationship between the cardiac cyclic variation of ultrasonic tissue integrated backscatter of left ventricular myocardium(LV-CVIB) and the left ventricular diastolic function.METHODS The CVIB of ant-septum wall and posterior wall on the left ventricular short-axis both at papillary level (LV-SAX-PM) and at mitral chordae tendineae level (LV-SAX-CT) were measured by Agilent-5500 ultrasonic instrument in 18 patients with hypertrophic cardiomyopathy (HCM group) and 22 normal people(controls). Pulmonary vein flow graph and mitral value flow graph were measured in both groups at the same time. We calculated the average CVIB(ACVIB) and performed a regression analysis on the relationship between the ACVIB and the left ventricular diastolic function data.RESULTS The ACVIB is significantly correlated with most of the left ventricular diastolic function data as follows:① correlated with Ve and Ve/Va(r=0.58 and 0.65,allP<0.01);②correlated with DT(r=-0.83,P< 0. 01);③correlated with Vd and Vs/Vd (r = 0. 57 and - 0. 68, allP< 0. 01). CONCLUSION The decreasing of ACVIB is significantly correlated with the decreasing of the left ventricular diastolic function in patients with HCM.
