摘要:
慢性心力衰竭(CHF)是一种常见的慢性心血管疾病，是各种心血管疾病进展的最后阶段, 随着对心血管疾病治疗方法的改进和人口寿命的延长，CHF的发生率逐年增加，已成为主要的公共卫生问题。建立医院—社区—病人三位一体的全方位管理模式，能够有效提高改善患者生活质量，降低医疗费用，减少再住院率，提高整体治疗效果。
Abstract:
Chronic heart failure (CHF), a common chronic cardiovascular disease, is the last stage of various cardiovascular diseases. With increased life expectancy of the population, the incidence of chronic heart failure increases each year and has become a major public health problem. The establishment of an integrated hospital-community-patient-based trinity management can effectively enhance the patients quality of life, reduce health care costs, lower readmission rates and improve the overall treatment effect.
