摘要:
目的 观察不同类型心绞痛患者血浆内皮素(ET ) 的变化, 探讨它与冠脉病史和射血分数的关系。方法 30 例心绞痛患者, 按照临床表现和冠脉狭窄程度分为: 稳定型心绞痛(SAP) 和不稳定型心绞痛(UAP) 组; 轻度狭窄、中度狭窄和重度狭窄组。结果 UAP 血浆ET 显著高于SA P 和正常对照组(P < 0. 01) , 而SAP 与正常对照组无差异(P > 0. 05) ; 随着冠脉狭窄程度加重, 血浆ET 有增高趋势。重度狭窄组显著高于轻度和中度狭窄组(P < 0. 05或P< 0. 01)。结论 血浆ET 水平与冠脉病变积分呈正相关( r= 0. 524, P < 0. 01) , 而与左室射血分数呈负相关( r= -01496, P < 0. 01)。
Abstract:
AIM To assess the changes of plasma ET level and relation shop with the severity of left ventriuclar ejection fraction (LVEF ) and clinical presentation in varied coronary artery disease(CAD ). METHODS 30 cases with either stable angina (SAP ) or unstab leangina (UAP) , were performed with coronary angiography (CAG). According to the feature of angina pectoris and unequivocal evidence of arterial narrowing, the patients were classified into: ①SAP group and UAP group and ② mild, middle-and severe-narrowed artery groups. The samples were drawn before CAG. RESULTS The levels of plasma ET were higher in the UAP group compared with the control group (P < 0. 01) , but the plasma ET level in the SAP group was nosignificant difference with the control group (P > 0. 05). The plasma ET levels increased in accordance with the degree of the coronary artery narrowing. The plasma ET level of the patients with severe-narrowed artery was higher than that of the mild and middle-narrowed (P < 0. 05, or P < 0. 01). Them iddle2narrow ed group w as h igher than that of the con t ro l group (P < 0. 01). The p lasm a ET level tended to increase in m iddle2narrow ed group , bu t the change w as no sign if ican t stat ist ically. The p lasm a ET level show ed a po sit ive co rrelat ion w ith the co ronary abno rm ality index ( r = 0. 524, P < 0. 01) and negat ive co rrelat ion w ith LV EF ( r = - 0. 496, P < 0. 01). CONCLUSION The levels of p lasm a ET w ill be associated w ith the clin ical p resen tat ion, and the degree of co ronary artery narrow ing. The mo re co ronary vessels affected as w ell as the wo rse ven t ricu lar funct ion, the h igher the p lasm a ET levels w ere. It indicates that the p lasm a ET can ref lect the severity of CAD and be u sed as an indicato r fo r the severity and the p rogno sis of the CAD.
