摘要:
目的 探讨胰岛素在心肌缺血/再灌注( I/R ) 期间对心肌细胞的保护作用。方法 制备兔I/R 模型并将其随机分为3组, 分别于缺血前30 min 予以葡萄糖2胰岛素2钾(GIK) (葡萄糖150 g/L , 胰岛素100 U/L , 钾80 mmo l/L )、胰岛素(100 U/L )、生理盐水静脉注射(1. 2 ml/h ) , 然后实施40 min 的心肌局部缺血和3h的再灌注。采集并处理心肌血流动力学指标, 检验乳酸脱氢酶(LDH)、肌酸激酶(CK) 活性。3h 后, 采用E-vans 蓝-TTC 法测量心肌梗死范围。结果 与对照组比较, GIK 及胰岛素组在血流动力学(LV EDP, ±dp/d tmax) , CK,LDH、梗死范围测量等均表现为损伤减轻, 且这两组间未见明显差异。结论 在I/R 的过程中, GIK 合剂除了提供能量外, 其中的胰岛素也发挥了重要的心肌保护作用。
Abstract:
AIM To evaluate the role of insulin in myocardial ischemia reperfusion injury.METHODS 24 rabbits are divided into three group srandomly and myocardial ischemia reperfusion model in rabbits was made. Rabbits were subjected to 30 min of regional myocardial ischemia and 3 h of reperfusion. GIK (glucose 150 g/L , insulin 100 U/L , potassium 80 mmol/L ) ,insulin(100 U/L ) , Vehicle (9 g/L NaCl)were given i. v. 30 min before ischemia. The index of myocardial function was recorded and analyzed. The activity of CK and LDH were detected.Infarced sizes in heart were deterimined by dual staining with Evans-blue and TTC. RESULTS Incomparison with control group , all indexes related to injury, namely LVEDP, ±dp/dtmax , CK,LDH and the ratio of myocardial necrosis area, show injure at tenuation in GIK and insulin group . CONCLUSION Du ing m ocardial ischemia reperfusion, GIK not only provides energy,but also insulin in it plays a critical role in protecting ischemic cardiocytes.
