摘要:
目的 探讨冠心病心电图P/P-R段比值与多普勒左室舒张功能参数的相关性及其意义。方法 研究对象为52例冠心病患者, 心电图测量P/P-R 比值。采用多普勒超声心动图测量舒张功能指标, 包括E 波加速时间(ACCT)、E波减速时间(DECT )、E 峰值速度(E)、A 峰值速度(A )、E/A 比值、E 波流速积分(ETVI)、1/3 舒张期流速积分(TVI 1/3)、全舒张期流速积分(TOTALTVI) 以及E 波充盈分数(RFF) 进行了统计分析。结果 P/P-R 段比值与二尖瓣血流频谱流速及流速积分相关良好, 冠心病P/P-R 段比值增大, 流速及流速积分参数减低, 对比有显著或非常显著差异(P<0.05, P<0.01)。结论 P/P-R 段比值测量简单易行, 可靠性较强, 对评价冠心病左室舒张功能有一定的应用价值。
Abstract:
AIM To study the correlation between P/P-R segment ratio of electrocardiogram and Doppler parameter of left ventricular diastolic function in patient with coronary heart disease (CHD).METHODS 52 cases of CHD were observed in this group. P-wave time of electrocardiogram divided by P-R segment was P/P-R segment ratio. Doppler ultrasound cardiogram was adopted to get the frequency spectrum parameters of mitral blood flow and to measure the diastolic function indexes,including E-wave accelerating time(ACCT),E-wave decelerating time (DECT),E-peak value speed(E),A-Peak value speed(A),E/A ratio,E-wave flow rate integral (ETVI),flow rate integral of one third of diastolic phase(TVI 1/3),holodiastolic flow rate integral(TOTAL,TVI),E-wave filling fraction(RFF).RESULTS The P/P-R segment ratio was well correlated with the frequency spectrum parameters of mitral blood flow and the flow rate integral. In the case of CHD,the parameters of flow rate and flow rate integral decreased as P/P-R segment ratio increased,the contrast remarkable or quite remarkable(P<0.05,P<0.01).CONCLUSION The P/P-R segment ratio is reliable and is easy to measure,therefore it is valuable in evaluating left ventricular diastolic function in CHD patient.
