目的 研究内皮素(ET)和一氧化氮(NO)与慢性心力衰竭的关系,并探讨不同剂量培哚普利在心力衰竭治疗中对ET和NO的影响。方法 慢性心力衰竭患者74例(其中NYHAⅡ级28例、Ⅲ级24例、Ⅳ级22例)和正常对照者24例,测定其基础血浆ET和NO值。心力衰竭患者随机分成小剂量培哚普利组与较大剂量组,治疗6周后,再次测定ET和NO值。结果 血浆中ET、NO含量随着心衰的严重程度而逐渐增高,并与Boston心衰积分(BSS)呈正相关(P<0.01),与左室射血分数(LVEF)呈负相关(P<0.01);经抗心衰治疗后两组患者ET、NO显著降低(P<0.01),其中较大剂量组治疗后ET、NO较小剂量组显著降低(P<0.01)。结论 心力衰竭患者的血浆ET和NO的水平与心力衰竭的严重程度密切相关,较大剂量的血管紧张素转换酶抑制剂(ACEI)能显著降低血浆ET、NO值,更好地改善心力衰竭。
Abstract:
AIM To study the correlation of the endothelin (ET) and nitric oxide(NO) with chronic heart failure and the effect of various doses of Perindopril on ET and NO in patients with chronic heart failure. METHODS Seventyfour patients with heart failure (28 patients with NYHA I, 24 patients with NYHA II, 22 patients with NYHA Ⅳ)were randomly divided into lowdose and highdose Perindopril groups. The plasma levels of ET and NO were measured just before and 6 weeks after the therapy. The basic plasma levels of ET and NO were also measured in 24 healthy subjects which serve as the control group. RESULTS The levels of ET and NO showed a positive correlation with the severity and Boston scoring system(BSS) (P<0.01) but a negative correlation with the left ventricular ejection fraction(LVEF)(P<0.01). The levels of ET and NO were reduced 6 weeks after the therapy in both groups(P<0.01). The reduced levels of ET and NO of the highdose group were obviously lower than those of the lowdose group(P<0.01). CONCLUSION The plasma levels of ET and NO were correlated with the severity of the chronic heart failure. Highdose Perindopril could reduce plasma ET and NO, thus improving the cardiac function.
