目的 研究在短期快速肺静脉起搏下,易诱发房颤(AF)的兔肺静脉心肌细胞有无急性电重构改变。方法 建立兔肺静脉源性AF的动物模型,应用程序刺激的方法测定和计算肺静脉远、近两端的有效不应期之差(ΔERP);并通过全细胞膜片钳技术记录钙离子流变化。结果 在同样刺激条件下,不是所有实验动物均可诱发出AF。快速肺静脉起搏和AF时均引起肺静脉远、近两端心肌细胞ERP之差值(ΔERP)减小,同时在AF持续过程中还可引起钙电流减小。结论 通过短期快速肺静脉刺激,发现易触发AF的肺静脉在AF发生和持续时,其肺静脉心肌细胞有急性电重构的存在,L型钙电流减小可能是短期AF导致肺静脉心肌细胞发生急性电重构的离子基础之一。
Abstract:
AIM To study whether acute electrophysiological remodeling occurred in pulmonary vein cardiomyocytes during easilyinduced atrial fibrillation (AF) caused by shortterm rapid pacing of pulmonary vein. METHODS Rabbit models of pulmonary vein atrial fibrillation were established. The differences of effective refractory period (ΔERP) between distal and proximal part of pulmonary vein were determined and calculated by program stimulation and the changes of ionic calcium current was recorded by whole-cell patch clamp technique. RESULTS Under the same stimulus, atrial fibrillation was not induced in all the experimental animals. The shortterm rapid pacing of pulmonary vein and atrial fibrillation resulted in the decrease of ΔERP difference between distal and proximal part of pulmonary vein. With the maintenance of AF, Ltype calcium current (ICa·L) reduced in pulmonary vein cardiomyocytes. CONCLUSION Acute electrophysiological remodeling has been detected in pulmonary vein cardiomyocytes of easilyinduced AF during initiation and maintenance of AF caused by shortterm rapid pacing of pulmonary vein. The decrease of ICa·L may contribute to ionic basis of acute electrical remodeling of pulmonary vein cardiomyocytes caused by shortterm AF.
