摘要:
目的 探讨超敏C反应蛋白(hsCRP)、脂蛋白(α) [Lp(α)]和脂联素联合检测对冠心病的诊断价值。方法 对161例疑诊冠心病的患者进行冠状动脉造影检查,根据造影结果分为冠心病组和非冠心病组,收集5 ml无抗凝全血标本,分离血清,利用悬浮芯片同时检测同一标本hsCRP、Lp(α)和脂联素3个指标。结果 冠心病组hsCRP、Lp(α)测定值均显著高于非冠心病组(P<0.01),而冠心病组脂联素测定值显著低于非冠心病组(P<0.01);3个指标集成检测进行平行试验分析时灵敏度(90%)显著高于单个标志物检测的灵敏度[hsCRP 65%、Lp(α)74%、脂联素58%,P<0.01],进行系列试验分析时特异度(92%)显著高于单个标志物检测的特异度(hsCRP 85%、Lp(α)69%、脂联素75%,P<0.01)。结论 集成检测hsCRP、Lp(α)和脂联素能更准确诊断冠心病,有效的减少漏诊与误诊。
Abstract:
AIM To study the value of integrated detection of serum highsensitivity Creactive protein (hsCRP), lipoprotein(α) [Lp(α)] and adiponectin in diagnosis of coronary heart disease (CHD). METHODS One hundred and sixtyone suspected cases of CHD underwent coronary angiography and were classified into CHD group and nonCHD group. 5mL peripheral blood were drawn from all the subjects and the serum samples were separated. hsCRP, Lp(α) and adiponectin were evaluated and analyzed in both groups by suspension chip. RESULTS The concentration of hsCRP and Lp(α) was significantly higher in CHD patients than that in nonCHD subjects (P<0.01) and the concentration of adiponectin was significantly lower in CHD patients compared with that in nonCHD subjects (P<0.01). By parallel tests, the sensitivity of integrated detection all the three markers (0.90) was higher than that of single marker detection of either hsCRP (0.65) or Lp(α) (0.74) or adiponectin (0.58) (P<0.01). By serial tests, the specificity of integrated detection all the three markers (0.92) was higher than that of single marker detection of either hsCRP (0.85) or Lp(α) (0.69) or adiponectin (0.75) (P<0.01). CONCLUSION Integrated detection of three markers of hsCRP, Lp(α) and adiponectin is more accurate and reliable in the diagnosis of CHD.
