小剂量吸入NO降低自主呼吸犬缺氧性肺动脉高压X(PDF)

摘要:
本文观察国内配制一氧化氮(NO) 气体对缺氧性肺动脉高压的作用。使9条麻醉犬吸入氧浓度从0.36±0.09 降至0.08±0.02, 然后吸入90±8 ppm NO , 结果发现缺氧后, 平均肺动脉压、肺循环阻力、平均股动脉压和心输出量显著增加, 呼吸及心率增快, 血气分析示过度通气; 吸入NO 后, 增高的平均肺动脉压和肺循环阻力显著下降(P<0.05) , 而平均股动脉压、心输出量、体循环阻力、心率无明显变化; NO 吸入10 min 时血中高铁血红蛋白浓度从0.3±0.1% 升至0.7±0.5%。说明如控制吸入NO 剂量及严密监测, 可将该气体试用于临床, 从而选择性降低肺动脉高压。
Abstract:
The effect of nitric oxide produced by Beijing Beifen Scienti ic Gas Co.on anoxic pulmonary hypertension was ob served in nine anesthetized dogs at spontaneous breathing in this study.When FiO2 was decreased from 36±9% to 8±2% and 90±8 ppm NO was given. The results showed that after hypoia, the mean pulmonary arterial p ressure(MPAP), the pulmonary vascular resistance (PVR ) , the mean blood pressure (MBP) , the cardiac output(CO),the respiratory rate and the heart rate increased significantly; blood gas analysis showed hyperventilation.When NO was inhaled, increased MPAP and PVR were decreased (P< 0.05),but MBP、CO、the heart rate and the systemic vascular resistance had no obvious changes (P> 0.05).Methaemoglobin increased from 0.3±0.1% to 0.7±0.5%(P<0.05) 10 min after inhalation of NO. It is proved that the gas could be used for pat ients with pulmonary hypertension to selectively dilate pulmonary vasculature at low dose inhalation of NO and under close monitoring.
