目的: 探讨正常成年人群长期吸烟者心脏解剖结构和功能变化及可能的参与因素。方法: 以超声心动图检查一组确证无心血管系统疾病及其它相关严重疾病的30例长年吸烟和52例除性别构成外, 年龄、体重指数、血压水平相匹配但不吸烟的正常健康成人心脏结构和功能指标; 以协方差分析校正性别构成不均衡, 以及年龄、收缩压、舒张压、体重指数可能的影响后进行对比分析和相关分析。结果和结论: 长期吸烟者一些心脏解剖及收缩功能指标与吸烟程度间存在显著性相关关系, 与不吸烟者相比, 部分指标差异也有显著性。但考虑性别、年龄、体重指数, 血压水平可能的影响并予以校正后, 吸烟者心脏测值仍有意义的变化有: 与吸烟程度相关的左房内径增大, 搏出量指数及左室射血分数下降, 平均肺动脉压上升。大血管和心室内径及室壁厚度变化不大; 反映左室舒张功能的指标与不吸烟者无异, 与吸烟程度也无相关性。有一些在对比分析时差异不一定有显著性的因素可能影响这些指标的变化, 如血压水平、年龄、体重指数等, 应注意予以考虑和校正。
Abstract:
Object: The purpose of this study was to assess the changes of cardiac structures,functions and relative factors in long-term smokers. Method: Indices of cardiac structures and functions were studied by echocardiography in a series of 30 long-term smokers(S group)without evidence of any cardiovascular disease or other severe diseases and compared with those of 52 normal nonsmokers(NS group)matched for age, Body mass index(BMI)and the level of blood pressure after adjusting for effects of gender,age,SBP, DBP and BMI by method of covariance analysis . The lineal correlations between the cardiac indices and the number of cigrette/day analysis were performed too.Results and Conclusions: It was found that statistically significant correlations exist between some indices of cardiac structures and systolic function in S group,and the differences of these indices between s group and Ns group were also statistically significant.The changes of indices remained statistically significant differences after adjusting effects of gender,age, BMI and the level of blood pressure including increase of left atrial dimension, decrease of strore volume index and left ventricle ejection fraction and elevation of mean pulmonary artery pressure associated with smoking. No significant changes of major vascular and ventricular chamber dimensions and ventricular wall thickness were detected. The indices of left ventricular diastolic function were not statistically different between two groups, which did not correlate with the number of cigarette per day. Some physiological factors such as age, blood pressure, BMI would affect the changes of some cardiac indices even if they were not significant difference in comparative analysis. Therefore these factors should be taken into account and corrected in analysis.
