摘要:
目的:探讨心脏肌钙蛋白I(cTnI)对急性心肌梗死(AMI)患者心功能预后的判断价值。方法:AMI患者34例(溶栓再通组18例,未溶栓组16例),每例患者采用序列采血测定血清cTnI,并检测1月末左心室射血分数(LVEF)。结果:AMI患者血清的ΣcTnI与1月末LVEF呈负相关(P<0.05)。溶栓再通患者的cTnI峰值浓度与 1月末LVEF进行相关分析,有明显的负相关(P<0.05),未溶栓患者的cTnI峰值浓度与1月末LVEF无明显相关(P>0.05),溶栓再通患者cTnI峰值浓度明显高于未溶栓患者(P<0.01)。结论:AMI患者的ΣcTnI和溶栓再通患者的峰值浓度均可作为判断左心功能的预后指标。
Abstract:
AIM: To explore the predictable value of serum cardiac troponin I (cTn I) in acute myocardial infarction (AMI) patients with left vetricular function and prognosis. METHODS: The patient population consisted of 34 AMI patients (18 receiving reperfusion after venous thrombolysis and 16 non-thrombolysis). Blood samples were collected serially in AMI patients, and the left vetricular functionand prognosis were checked at the end of month. RESULTS: Serum cumulative cTn I concentraton (ΣcTn I) showed negative correlation with LVEF at the endofone month; Serum cTn I peak value in patient with reperfusion after thrombolysis showed significantly negative correlation with LVEF at the endofone month (r=-0.496,P<0.05); serum cTn I peak value in patient with no thrombolysis showed no correlation with LVEF at the endofone month (P> 0.05). Serum cTn I peak value in patient with reperfusion after thrombolysis were higher than no thrombolysis (P<0.01). CONCLUSION: Σ cTn I of AMI patients and the peak of cTn I in reperfusion patients is a reference index fo restimating left vetricular function and prognosis.
